Our die() code was based off a very old x86 version. Update it to
mirror the current x86 code.

This patch implements a back-end cpuidle driver for pSeries
based on pseries_dedicated_idle_loop and pseries_shared_idle_loop
routines. The driver is built only if CONFIG_CPU_IDLE is set. This
cpuidle driver uses global registration of idle states and
not per-cpu.

This patch provides cpu_idle_wait() routine for the powerpc
platform which is required by the cpuidle subsystem. This
routine is required to change the idle handler on SMP systems.
The equivalent routine for x86 is in arch/x86/kernel/process.c
but the powerpc implementation is different.
cpuidle_disable variable is to enable/disable cpuidle
framework if power_save option is set during the boot
time.

Replace all remaining callers of alloc_maybe_bootmem with
zalloc_maybe_bootmem. The callsite in pci_dn is followed with a
memset to clear the memory, and not zeroing at the other callsites
in the celleb fake pci code could lead to following uninitialized
memory as pointers or even freeing said pointers on error paths.

In old kernels, NET_SKB_PAD was defined to 16.
Then commit d6301d3dd1 (net: Increase default NET_SKB_PAD to 32), and
commit 18e8c134f4 (net: Increase NET_SKB_PAD to 64 bytes) increased it
to 64.
While first patch was governed by network stack needs, second was more
driven by performance issues on current hardware. Real intent was to
align data on a cache line boundary.
So use max(32, L1_CACHE_BYTES) instead of 64, to be more generic.
Remove microblaze and powerpc own NET_SKB_PAD definitions.

For performance reasons we are about to change ISYNC_ON_SMP to sometimes be
lwsync. Now that the macro name doesn't make sense, change it and LWSYNC_ON_SMP
to better explain what the barriers are doing.

For some obscure reason, we only set init_bootmem_done after initializing
bootmem when NUMA isn't enabled. We even document this next to the declaration
of that global in system.h which of course I didn't read before I had to
debug why some WIP code wasn't working properly...
This patch changes it so that we always set it after bootmem is initialized
which should have always been the case... go figure !

We're currently choking on mem=4g (and above) due to memory_limit
being specified as an unsigned long. Make memory_limit
phys_addr_t to fix this.

Randomise the lower bits of the stack address. More randomisation is good for
security but the scatter can also help with SMT threads that share an L1. A
quick test case shows this working:

After commit 598056d5af ("[POWERPC] Fix
rmb to order cacheable vs. noncacheable"), rmb() becomes a sync
instruction, which is needed to order cacheable vs noncacheable loads.
However smp_rmb() is #defined to rmb(), and smp_rmb() can be an
lwsync.
This restores smp_rmb() performance by using lwsync there and updates
the comments.

Change 2d1b202762 ("powerpc: Fixup
lwsync at runtime") removed __SUBARCH_HAS_LWSYNC, causing smp_wmb to
revert back to eieio for all CPUs. This restores the behaviour
intorduced in 74f0609526 ("powerpc:
Optimise smp_wmb on 64-bit processors").

Followed by a few documentation/comment fixups and a couple of places
where <asm-powepc/...> was being used explicitly. Of the latter only
one was outside the arch code and it is a driver only built for powerpc.
