The only overlap between the block open flags mapped into the fmode_t and
other uses of fmode_t are FMODE_READ and FMODE_WRITE. Define a new
blk_mode_t instead for use in blkdev_get_by_{dev,path}, ->open and
->ioctl and stop abusing fmode_t.

The current interface for exclusive opens is rather confusing as it
requires both the FMODE_EXCL flag and a holder. Remove the need to pass
FMODE_EXCL and just key off the exclusive open off a non-NULL holder.
For blkdev_put this requires adding the holder argument, which provides
better debug checking that only the holder actually releases the hold,
but at the same time allows removing the now superfluous mode argument.

Add a new blk_holder_ops structure, which is passed to blkdev_get_by_* and
installed in the block_device for exclusive claims. It will be used to
allow the block layer to call back into the user of the block device for
thing like notification of a removed device or a device resize.

There is no need to have the functions blkif_get_x86_32_req() and
blkif_get_x86_64_req() in a header file, as they are used in one place
only.
So move them into the using source file and drop the inline qualifier.
While at it fix some style issues, and simplify the code by variable
reusing and using min() instead of open coding it.
Instead of using barrier() use READ_ONCE() for avoiding multiple reads
of nr_segments.

grants") made a field of blkback, which was a place for saving only the
negotiation result, to be used for yet another purpose: caching of the
'feature_persistent' parameter value. As a result, the advertisement,
which should follow only the parameter value, becomes inconsistent.
This commit fixes the misuse of the semantic by making blkback saves the
parameter value in a separate place and advertises the support based on
only the saved value.

In some use cases[1], the backend is created while the frontend doesn't
support the persistent grants feature, but later the frontend can be
changed to support the feature and reconnect. In the past, 'blkback'
enabled the persistent grants feature since it unconditionally checked
if frontend supports the persistent grants feature for every connect
('connect_ring()') and decided whether it should use persistent grans or
not.
However, commit aac8a70db2 ("xen-blkback: add a parameter for
disabling of persistent grants") has mistakenly changed the behavior.
It made the frontend feature support check to not be repeated once it
shown the 'feature_persistent' as 'false', or the frontend doesn't
support persistent grants.
This commit changes the behavior of the parameter to make effect for
every connect, so that the previous workflow can work again as expected.

Persistent grants feature can be used only when both backend and the
frontend supports the feature. The feature was always supported by
'blkback', but commit aac8a70db2 ("xen-blkback: add a parameter for
disabling of persistent grants") has introduced a parameter for
disabling it runtime.
To avoid the parameter be updated while being used by 'blkback', the
commit caches the parameter into 'vbd->feature_gnt_persistent' in
'xen_vbd_create()', and then check if the guest also supports the
feature and finally updates the field in 'connect_ring()'.
However, 'connect_ring()' could be called before 'xen_vbd_create()', so
later execution of 'xen_vbd_create()' can wrongly overwrite 'true' to
'vbd->feature_gnt_persistent'. As a result, 'blkback' could try to use
'persistent grants' feature even if the guest doesn't support the
feature.
This commit fixes the issue by moving the parameter value caching to
'xen_blkif_alloc()', which allocates the 'blkif'. Because the struct
embeds 'vbd' object, which will be used by 'connect_ring()' later, this
should be called before 'connect_ring()' and therefore this should be
the right and safe place to do the caching.

Just use a non-zero max_discard_sectors as an indicator for discard
support, similar to what is done for write zeroes.
The only places where needs special attention is the RAID5 driver,
which must clear discard support for security reasons by default,
even if the default stacking rules would allow for it.

Pass the block_device and operation that we plan to use this bio for to
bio_alloc to optimize the assignment. NULL/0 can be passed, both for the
passthrough case on a raw request_queue and to temporarily avoid
refactoring some nasty code.
Also move the gfp_mask argument after the nr_vecs argument for a much
more logical calling convention matching what most of the kernel does.

GENHD_FL_CD marks a gendisk as a vaguely CD-ROM like device.
Besides being used internally inside of sunvdc.c an xen-blkfront it
is used by xen-blkback as a hint to claim a device exported to a
guest is a CD-ROM like device. Just check for disk->cdi instead
which is the right indicator for "real" CD-ROM or DVD drivers. This
will miss the paravirtualized guest drivers, but those make little
sense to report anyway.

Prior to commit 4a8c31a1c6 ("xen/blkback: rework connect_ring() to avoid
inconsistent xenstore 'ring-page-order' set by malicious blkfront"), the
behaviour of xen-blkback when connecting to a frontend was:
- read 'ring-page-order'
- if not present then expect a single page ring specified by 'ring-ref'
- else expect a ring specified by 'ring-refX' where X is between 0 and
1 << ring-page-order
This was correct behaviour, but was broken by the afforementioned commit to
become:
- read 'ring-page-order'
- if not present then expect a single page ring (i.e. ring-page-order = 0)
- expect a ring specified by 'ring-refX' where X is between 0 and
1 << ring-page-order
- if that didn't work then see if there's a single page ring specified by
'ring-ref'
This incorrect behaviour works most of the time but fails when a frontend
that sets 'ring-page-order' is unloaded and replaced by one that does not
because, instead of reading 'ring-ref', xen-blkback will read the stale
'ring-ref0' left around by the previous frontend will try to map the wrong
grant reference.
This patch restores the original behaviour.

The fix for XSA-365 zapped too many of the ->persistent_gnt[] entries.
Ones successfully obtained should not be overwritten, but instead left
for xen_blkbk_unmap_prepare() to pick up and put.

It's often inconvenient to use BIO_MAX_PAGES due to min() requiring the
sign to be the same. Introduce bio_max_segs() and change BIO_MAX_PAGES to
be unsigned to make it easier for the users.

The function uses a goto-based loop, which may lead to an earlier error
getting discarded by a later iteration. Exit this ad-hoc loop when an
error was encountered.
The out-of-memory error path additionally fails to fill a structure
field looked at by xen_blkbk_unmap_prepare() before inspecting the
handle which does get properly set (to BLKBACK_INVALID_HANDLE).
Since the earlier exiting from the ad-hoc loop requires the same field
filling (invalidation) as that on the out-of-memory path, fold both
paths. While doing so, drop the pr_alert(), as extra log messages aren't
going to help the situation (the kernel will log oom conditions already
anyway).

In particular -ENOMEM may come back here, from set_foreign_p2m_mapping().
Don't make problems worse, the more that handling elsewhere (together
with map's status fields now indicating whether a mapping wasn't even
attempted, and hence has to be considered failed) doesn't require this
odd way of dealing with errors.

In order to support the possibility of per-device event channel
settings (e.g. lateeoi spurious event thresholds) add a xenbus device
pointer to struct irq_info() and modify the related event channel
binding interfaces to take the pointer to the xenbus device as a
parameter instead of the domain id of the other side.
While at it remove the stale prototype of bind_evtchn_to_irq_lateeoi().

When xen_blkif_disconnect() is called, the kernel thread behind the
block interface is stopped by calling kthread_stop(ring->xenblkd).
The ring->xenblkd thread pointer being non-NULL determines if the
thread has been already stopped.
Normally, the thread's function xen_blkif_schedule() sets the
ring->xenblkd to NULL, when the thread's main loop ends.
However, when the thread has not been started yet (i.e.
wake_up_process() has not been called on it), the xen_blkif_schedule()
function would not be called yet.
In such case the kthread_stop() call returns -EINTR and the
ring->xenblkd remains dangling.
When this happens, any consecutive call to xen_blkif_disconnect (for
example in frontend_changed() callback) leads to a kernel crash in
kthread_stop() (e.g. NULL pointer dereference in exit_creds()).
This is XSA-350.

Some code does not directly make 'xenbus_watch' object and call
'register_xenbus_watch()' but use 'xenbus_watch_path()' instead. This
commit adds support of 'will_handle' callback in the
'xenbus_watch_path()' and it's wrapper, 'xenbus_watch_pathfmt()'.

Now that the hd_struct always has a block device attached to it, there is
no need for having two size field that just get out of sync.
Additionally the field in hd_struct did not use proper serialization,
possibly allowing for torn writes. By only using the block_device field
this problem also gets fixed.

Persistent grants feature provides high scalability. On some small
systems, however, it could incur data copy overheads[1] and thus it is
required to be disabled. But, there is no option to disable it. For
the reason, this commit adds a module parameter for disabling of the
feature.

In order to reduce the chance for the system becoming unresponsive due
to event storms triggered by a misbehaving blkfront use the lateeoi
irq binding for blkback and unmask the event channel only after
processing all pending requests.
As the thread processing requests is used to do purging work in regular
intervals an EOI may be sent only after having received an event. If
there was no pending I/O request flag the EOI as spurious.

Patch series "mm: cleanup usage of <asm/pgalloc.h>"
Most architectures have very similar versions of pXd_alloc_one() and
pXd_free_one() for intermediate levels of page table. These patches add
generic versions of these functions in <asm-generic/pgalloc.h> and enable
use of the generic functions where appropriate.
In addition, functions declared and defined in <asm/pgalloc.h> headers are
used mostly by core mm and early mm initialization in arch and there is no
actual reason to have the <asm/pgalloc.h> included all over the place.
The first patch in this series removes unneeded includes of
<asm/pgalloc.h>
In the end it didn't work out as neatly as I hoped and moving
pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require
unnecessary changes to arches that have custom page table allocations, so
I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local
to mm/.
This patch (of 8):
In most cases <asm/pgalloc.h> header is required only for allocations of
page table memory. Most of the .c files that include that header do not
use symbols declared in <asm/pgalloc.h> and do not require that header.
As for the other header files that used to include <asm/pgalloc.h>, it is
possible to move that include into the .c file that actually uses symbols
from <asm/pgalloc.h> and drop the include from the header file.
The process was somewhat automated using
