9fb7a5f77b e1000e: minor re-order of #include files
The recent commit a6b7a407 when back-ported to the out-of-tree e1000e
driver caused a compilation error on older kernels which required a
re-ordering of the #include files.  This cosmetic patch syncs the two
drivers for easier maintainability.

945a51517c intel drivers: repair missing flush operations
after review of all intel drivers, found several instances where
drivers had the incorrect pattern of:
memory mapped write();
delay();

which should always be:
memory mapped write();
write flush(); /* aka memory mapped read */
delay();

explanation:
The reason for including the flush is that writes can be held
(posted) in PCI/PCIe bridges, but the read always has to complete
synchronously and therefore has to flush all pending writes to a
device.  If a write is held and followed by a delay, the delay
means nothing because the write may not have reached hardware
(maybe even not until the next read)

3ebfc7c9a6 e1000e: Clear host wakeup bit on 82577/8 without touching PHY page 800
The Host Wakeup Active bit in the PHY Port General Configuration register
(page 769 register 17) must be cleared after every PHY reset to prevent an
unexpected wake signal from the PHY. Originally, this was accomplished by
simply reading the PHY Wakeup Control register on page 800 which clears the
Host Wakeup Active bit as a side-effect. Unfortunately, a hardware bug on
the 82577 and 82578 PHY can cause unexpected behavior when registers on
page 800 are accessed while in gigabit mode.

This patch changes the remaining instances when the Host Wakeup Active bit
needs to be cleared while possibly in gigabit mode by accessing the Port
General Configuration register directly instead of accessing any register
on page 800.

2b6b168d52 e1000e: access multiple PHY registers on same page at the same time
Doing a PHY page select can take a long time, relatively speaking. This
can cause a significant delay when updating a number of PHY registers on
the same page by unnecessarily setting the page for each PHY access. For
example when going to Sx, all the PHY wakeup registers (WUC, RAR[], MTA[],
SHRAR[], IP4AT[], IP6AT[], etc.) on 82577/8/9 need to be updated which
takes a long time which can cause issues when suspending.

This patch introduces new PHY ops function pointers to allow callers to
set the page directly and do any number of PHY accesses on that page.
This feature is currently only implemented for 82577, 82578 and 82579
PHYs for both the normally addressed registers as well as the special-
case addressing of the PHY wakeup registers on page 800. For the latter
registers, the existing function for accessing the wakeup registers has
been divided up into three- 1) enable access to the wakeup register page,
2) perform the register access and 3) disable access to the wakeup register
page. The two functions that enable/disable access to the wakeup register
page are necessarily available to the caller so that the caller can restore
the value of the Port Control (a.k.a. Wakeup Enable) register after the
wakeup register accesses are done.

All instances of writing to multiple PHY registers on the same page are
updated to use this new method and to acquire any PHY locking mechanism
before setting the page and performing the register accesses, and release
the locking mechanism afterward.

Some affiliated magic number cleanup is done as well.

400484fa65 e1000e: do not schedule the Tx queue until ready
Start the Tx queue when the interface is brought up in e1000e_up() but do
not schedule the queue until link is up as detected in the watchdog task
which sets netif_carrier_on.

Also flush the descriptors and clean the Tx and Rx rings before resetting
the hardware when bringing the interface down otherwise there is a small
window where the watchdog task can be triggered with netif_carrier_off
and the Tx ring not yet empty which causes an additional and unnecessary
reset.

c5caf4825b e1000e: log when swflag is cleared unexpectedly on ICH/PCH devices
Since EXTCNF_CTRL.SWFLAG (used in the ownership arbitration of shared
resources, e.g. the PHY shared between the s/w, f/w, and h/w clients)
can be cleared by any of those clients, log a debug message when
software attempts to clear it and it is already cleared unexpectedly.
And since the swflag is cleared by a hardware reset, the driver does
not need to do that, but the mutex acquired when the bit is set must
still be cleared.

99730e4c13 e1000e: 82579 intermittently disabled during S0->Sx
When repeatedly cycling Sx->S0 states with the network cable unplugged,
the 82579 PHY may not initialize as expected and may require a full power
cycle to recover functionality to the device.  Workaround this by testing
access of the PHY registers after resuming; if that returns unexpected
results toggle the LANPHYPC signal to power cycle the PHY.

This is implemented in the new function e1000_resume_workarounds_pchlan()
which calls another new function, e1000_toggle_lanphypc_value_ich8lan(),
which has been created to reduce code duplication (same functionality
required by a previous workaround).  Also, e1000e_disable_gig_wol_ich8lan
is now e1000_suspend_workarounds_ich8lan to better reflect what it does.

d9b24135b9 e1000e: disable far-end loopback mode on ESB2
The ESB2 LAN includes a debug feature that enables far-end loopback (FELB)
of the SerDes/Kumeran interface.  This feature is activated when receiving
a sequence of symbols that includes a reserved codeword.  On a perfect
link, FELB would never be activated.  In the presence of bit errors, there
is a very small, but non-zero, probability of FELB being activated.

If the FELB is activated, the SerDes link becomes non-functional and must
be reset.  It could also corrupt the switching tables in the switch since
the ESB2 is transmitting packets with a different source MAC address.

This patch disables the FELB feature.

70c7160619 Add appropriate <linux/prefetch.h> include for prefetch users
After discovering that wide use of prefetch on modern CPUs
could be a net loss instead of a win, net drivers which were
relying on the implicit inclusion of prefetch.h via the list
headers showed up in the resulting cleanup fallout.  Give

9f728f53dd PCI/e1000e: Add and use pci_disable_link_state_locked()
Need to use it in _e1000e_disable_aspm.  This routine is used for error
recovery, where the pci_bus_sem is already held, and we don't want
pci_disable_link_state to try to take it again.  So add a locked variant

14ad2513ed net/igb/e1000/e1000e: more robust ethtool duplex/speed configuration
This makes sure that one cannot request a 99Mbps full-duplex and get a
100Mbps half-duplex configuration in return due to the way the
speed/duplex parameters are handled internally.

7073949720 ethtool: cosmetic: Use ethtool ethtool_cmd_speed API
This updates the network drivers so that they don't access the
ethtool_cmd::speed field directly, but use ethtool_cmd_speed()
instead.

For most of the drivers, these changes are purely cosmetic and don't
fix any problem, such as for those 1GbE/10GbE drivers that indirectly
call their own ethtool get_settings()/mii_ethtool_gset(). The changes
are meant to enforce code consistency and provide robustness with
future larger throughputs, at the expense of a few CPU cycles for each
ethtool operation.

All drivers compiled with make allyesconfig ion x86_64 have been
updated.

Tested: make allyesconfig on x86_64 + e1000e/bnx2x work

25db033881 ethtool: Use full 32 bit speed range in ethtool's set_settings
This makes sure the ethtool's set_settings() callback of network
drivers don't ignore the 16 most significant bits when ethtool calls
their set_settings().

dbf80dcbd8 e1000e: implement ethtool set_phys_id
Based on a patch from Stephen Hemminger <shemminger@vyatta.com>.

The new ethtool set_phys_id takes over controlling the LED for
identifying boards.  This fixes the lockout during that period.
For this device lots of extra infrastructure can also be removed by
using set_phys_id.

v2: - return blink frequency for parts that do not support blink in h/w
    - add blink_led function pointers for devices that do support blink
      in h/w to cleanup the test for this functionality

78cd29d5a9 e1000e: If ASPM L0s needs to be disabled, do it prior to enabling device
Based on a patch from Naga Chumbalkar <nagananda.chumbalkar@hp.com>:

If ASPM L0s needs to be disabled due to HW errata, do it prior to
"enabling" the device. This way if the kernel ever defaults its
aspm_policy to POLICY_POWERSAVE, then the e1000e driver will get a
chance to disable ASPM on the misbehaving device *prior* to calling
pci_enable_device_mem(). This will be useful in situations
where the BIOS indicates ASPM support on the server by clearing the
ACPI FADT "ASPM Controls" bit.

Note:
The kernel (2.6.38) currently uses the BIOS "default" as its aspm_policy.
However, Linux distros can diverge from that and set the default to
"powersave".

v2: o cleanup namespace pollution of e1000e_disable_aspm(),
    o fix type and initialization of the new aspm_disable_flag in a few
      functions, and
    o redefine FLAG2_DISABLE_ASPM_L0S to the first unused bit in
      adapter->flags2.

1bba4386ab e1000e: convert short duration msleep() to usleep_range()
With durations less than 20ms, the jiffies or legacy timer backed msleep()
may sleep ~20ms which might not be what the caller expects.  Instead, it
is recommended to use the hrtimers backed usleep_range().  For more, see
Documentation/timers/timers-howto.txt.  Issues reported by checkpatch.

In addition, remove unnecessary sleep in e1000e_write_nvm_spi().

6cc7aaed70 e1000e: do not toggle LANPHYPC value bit when PHY reset is blocked
When PHY reset is intentionally blocked on 82577/8/9, do not toggle the
LANPHYPC value bit (essentially performing a hard power reset of the
device) otherwise the PHY can be put into an unknown state.

1effb45cca e1000e: extend EEE LPI timer to prevent dropped link
The link can be unexpectedly dropped when the timer for entering EEE low-
power-idle quiet state expires too soon.  The timer needs to be extended
from 196usec to 200usec after every LCD (PHY) reset to prevent this from
happening.

5661aeb08e e1000e: extend timeout for ethtool link test diagnostic
With some PHYs supported by this driver, link establishment can take a
little longer when connected to certain switches.  Extend the timeout to
reduce the number of false diagnostic failures, and cleanup a code style
issue in the same function.

Based on a report and patch originally submitted by Prasanna Panchamukhi.

Use dev_kfree_skb_irq() in e1000_clean_jumbo_rx_irq() since this latter
function is called only in interrupt context.  This avoids "Warning:
kfree_skb on hard IRQ" messages.

4def99bbfd e1000e: disable broken PHY wakeup for ICH10 LOMs, use MAC wakeup instead
When support for 82577/82578 was added[1] in 2.6.31, PHY wakeup was in-
advertently enabled (even though it does not function properly) on ICH10
LOMs.  This patch makes it so that the ICH10 LOMs use MAC wakeup instead
as was done with the initial support for those devices (i.e. 82567LM-3,
82567LF-3 and 82567V-4).

713b3c9e4c e1000e: flush all writebacks before unload
The driver was not flushing all writebacks before unloading, possibly
causing memory to be written by the hardware after the driver had
reinitialized the rings.

This adds missing functionality to flush any pending writebacks and is
called in all spots where descriptors should be completed before the driver
begins processing.
