Removed tests which prevent transmission of ACL packets
when the device is in HCI_RAW mode. These tests verified that
there are ACL or LE links currently tracked by the HCI
connection manager. However, a HCI_RAW mode device does not
use the connection manager. In these circumstances, the connection
counts will be zero, and thus, transmitted ACL packets dropped.
The acl_num test is actually a vestige of a previous bulk URB
scheme that is no longer used by this driver (bulk URBs were not
started until at least one ACL connection was created). This was
incompatible with some endpoint implementations and was dropped -
see commit 43c2e57f94.
The utility of these tests is marginal - currently, the hci tx
scheduler cannot send an ACL or LE packet for an untracked connection
(except if the device is in HCI_RAW mode).
Lastly, no other transport layer driver enforces these same tests.

Reordered the BT ST channel registration, to make sure that the
event channel is registered before all others. This prevents a
situation where incoming events may cause kernel panic in the ST
driver if the event channel is not yet registered to handle
incoming events.In addition, the deregistration of the channels
was also modified, to be in the reversed order of the registration,
to allow the event channel to be the last one unregistered.

Disabling the bluetooth usb device embedded in (some) ThinkPads tends to
lead to errors like these:
btusb_bulk_complete: hci0 urb ffff88011b9bfd68 failed to resubmit (19)
btusb_intr_complete: hci0 urb ffff88011b46a318 failed to resubmit (19)
btusb_bulk_complete: hci0 urb ffff88011b46a000 failed to resubmit (19)
That is because usb_disconnect() doesn't "quiesces" pending urbs.
Disconnecting a device is a normal thing to happen so it's no big deal
that usb_submit_urb() returns -ENODEV. The simplest way to get rid of
these errors is to stop treating that return as an error. Trivial,
actually.
While we're at it, add comments to be explicit about the reasons we're
not complaining about -EPERM and -ENODEV.

This reverts commit b1c43f82c5.
It was broken in so many ways, and results in random odd pty issues.
It re-introduced the buggy schedule_work() in flush_to_ldisc() that can
cause endless work-loops (see commit a5660b41af: "tty: fix endless
work loop when the buffer fills up").
It also used an "unsigned int" return value fo the ->receive_buf()
function, but then made multiple functions return a negative error code,
and didn't actually check for the error in the caller.
And it didn't actually work at all. BenH bisected down odd tty behavior
to it:
"It looks like the patch is causing some major malfunctions of the X
server for me, possibly related to PTYs. For example, cat'ing a
large file in a gnome terminal hangs the kernel for -minutes- in a
loop of what looks like flush_to_ldisc/workqueue code, (some ftrace
data in the quoted bits further down).
...
Some more data: It -looks- like what happens is that the
flush_to_ldisc work queue entry constantly re-queues itself (because
the PTY is full ?) and the workqueue thread will basically loop
forver calling it without ever scheduling, thus starving the consumer
process that could have emptied the PTY."
which is pretty much exactly the problem we fixed in a5660b41af.
Milton Miller pointed out the 'unsigned int' issue.

In commit 86e09287e4, to reduce memory
usage, the functions of the ath3k module were rewritten to release the
firmware blob after it has been loaded (successfully or not).
The resuting code has some redundancy and the compiler can potentially
produce better code if we omit a function call that is unconditionally
executed in
,----
| if (ath3k_load_firmware(udev, firmware)) {
| release_firmware(firmware);
| return -EIO;
| }
| release_firmware(firmware);
|
| return 0;
| }
`----
It may also be argued that the rewritten code becomes easier to read,
and also to see the code coverage of the snippet in question.

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.
The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.
New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.
The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.
Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.
Many thanks to Julia Lawall for helping me learn to write a semantic
patch that does all this.

When driver is sending a command or data and the firmware is also
sending a sleep event, sometimes it is observed that driver will
continue to send the command/data to firmware right after processing
sleep event. Once sleep event is processed driver is not supposed to
send anything because firmware is in sleep state after that. Previously
interrupt processing was done in SDIO interrupt callback handler.
Now it is done in btmrvl driver main thread to solve the
cross-sending properly.

Instead of the old pcmcia_request_irq() interface, drivers may now
choose between:
- calling request_irq/free_irq directly. Use the IRQ from *p_dev->irq.
- use pcmcia_request_irq(p_dev, handler_t); the PCMCIA core will
clean up automatically on calls to pcmcia_disable_device() or
device ejection.
- drivers still not capable of IRQF_SHARED (or not telling us so) may
use the deprecated pcmcia_request_exclusive_irq() for the time
being; they might receive a shared IRQ nonetheless.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.
http://userweb.kernel.org/~tj/misc/slabh-sweep.py
The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Most of the irq_req_t typedef'd struct can be re-worked quite
easily:
(1) IRQInfo2 was unused in any case, so drop it.
(2) IRQInfo1 was used write-only, so drop it.
(3) Instance (private data to be passed to the IRQ handler):
Most PCMCIA drivers using pcmcia_request_irq() to actually
register an IRQ handler set the "dev_id" to the same pointer
as the "priv" pointer in struct pcmcia_device. Modify the two
exceptions (ipwireless, ibmtr_cs) to also work this waym and
set the IRQ handler's "dev_id" to p_dev->priv unconditionally.
(4) Handler is to be of type irq_handler_t.
(5) Handler != NULL already tells whether an IRQ handler is present.
Therefore, we do not need the IRQ_HANDLER_PRESENT flag in
irq_req_t.Attributes.

This patch adds support of USB autosuspend to the btusb driver.
If the device doesn't support remote wakeup, simple support based on
up/down is provided. If the device supports remote wakeup, additional
support for autosuspend while the interface is up is provided. This is
done by queueing URBs in an anchor structure and waking the device up
from a work queue on sending. Reception triggers remote wakeup.
The last busy facility of the USB autosuspend code is used. To close
a race between autosuspend and transmission, a counter of ongoing
transmissions is maintained.
Add #ifdefs for CONFIG_PM as necessary.

The error message "Removed only %u out of %u pkts" is printed when multiple
to be acked packets are queued.
if (i++ >= pkts_to_be_removed)
break;
This will break out of the loop and increase the counter i when
i==pkts_to_be_removed and the loop ends up with i=pkts_to_be_removed+1.
The following line
if (i != pkts_to_be_removed) {
BT_ERR("Removed only %u out of %u pkts", i, pkts_to_be_removed);
}
will then display the false message.
The counter i must not increase on the same statement.

Check that the result of kmalloc is not NULL before dereferencing it.
The patch also replaces kmalloc + memset by kzalloc.
The semantic match that finds this problem is as follows:

// <smpl>

expression *x;
identifier f;
constant char *C;

x = \(kmalloc\|kcalloc\|kzalloc\)(...);

The driver uses "u32" for alignment check and calculation which
works only on 32-bit system. It will crash the 64-bit system.
Replace "u32" with "unsigned long" to fix this issue.

The Marvell Bluetooth driver is full of Enter/Leave debug statements and
all of them are really pointless and only clutter the code. Seems to be
some left-overs when they ported the driver from Windows. For the Linux
driver lets remove these.

After fixing the driver to use skb_put properly for their HCI commands
only a few compiler warnings are left. Add proper casting for them.

The Marvell driver has some weird quirks on how to construct proper SKBs
with Bluetooth HCI commands. Fix it to use skb_put properly and also
use hci_opcode_pack instead of self-crafted macro.

For some reason the btmrvl_device struct has a name field that the SDIO
fills in, but then never ever uses again. That is totally pointless and
so just remove it.

Make the module description entries for the core and also the Marvell
SDIO driver match common practive inside the Bluetooth subsystem.

The Marvell Bluetooth SDIO driver has a really complicated concept on how
firmware names are assigned to specific device ids. Fix that by doing a
proper structure and assign it to the module device table.
And while at it fix various coding style weirdness that is still present
in this driver.

The Marvell core Bluetooth driver has various weird casting and unneeded
braces in its code that makes it hard to read. Remove all of these to
make the code a little bit simpler.

The Marvell Bluetooth driver has debugfs support and they are casting
like there is no tomorrow. Remove all of them and magically the code
becomes more readable.

Both header files of the Marvell Bluetooth driver are private anyway and
if the driver happens to include them twice or they create a circular
dependency then the driver needs fixing. So just remove both pointless
ifdefs.

The Makefile entry for the Marvell driver is broken when it comes to
handling the optional DEBUG_FS correctly. That must have been the reason
why they were using select in Kconfig in the first place. Fix this and
make it really optional.
Signed-off-by: Marcel Holtmann <marcel@holtmann.org>
The Marvell driver selects DEBUG_FS and FW_LOADER for its core driver
and that is pointless. Don't select DEBUG_FS since it is either enabled
or not and it is not for the driver to enable it. Also FW_LOADER is
only used within the SDIO driver and so just have that one select the
FW_LOADER option.

* Remove smp_lock.h from files which don't need it (including some headers!)
* Add smp_lock.h to files which do need it
* Make smp_lock.h include conditional in hardirq.h
It's needed only for one kernel_locked() usage which is under CONFIG_PREEMPT
This will make hardirq.h inclusion cheaper for every PREEMPT=n config
(which includes allmodconfig/allyesconfig, BTW)
